Rat Of Destiny - Steam Achievement

Considering the new rat themed update, I thought this was a good time to "Finish the Divine Path quest with a chosen who started as a rat."

There are different rats to choose from at the start of the game. Does it need to be a basic rat, or can I pick an enlightened or baby rat as well?

Unless it was changed with the recent DLC (which we have no clue about) you need to start with the regular basic rat.

I got the achievement by starting as a basic rat, but transformed into Healrat before finishing the Divine quest and I still got it.

Starting as the rat is the only requirement, you cab change its form, class and race all you wanted after you start the game with it as your chosen.
