Question about Cyber Slash Harpie Lady

When is it's effect:

"When a Spell/Trap card or effect is activated (Except during the Damage Step) (Quick Effect): You can target 1 monster your opponent controls or 1 "Harpie" monster you control; return it to hand"

supposed to take place?

When I try to use something like Karma cut or some normal spell (like one of the Harpie support spells) it doesn't take effect but when something like Shattered Axe is activated, Cyber Slash's effect can be activated.

Can someone help clarify this for me?

I believe this is a "when" vs "if" situation, meaning there must not be anything else happening after the s/t effect triggers.

I think your Duel Settings aren't properly set for the effect of Cyber Slash Harpie Lady. They should look like this:

Also the effect of Cyber Slash Harpie Lady has to be chained directly to a S/T card. If you activate a S/T card and your opponent chains first, you got chain-blocked and can't activate the effect of Cyber Slash Harpie lady in this chain anymore.

I'll keep that in mind. I do tend to forget about the If/When thing in this game sometimes.

It was still happening when there was nothing else going on in the chain, but changing the settings fixed my issue.

It targets either 1 monster in your opponent's Monster Zone or 1 face-up "Harpie" monster in your Monster Zone. It can be activated optionally by chaining directly to a Spell/Trap Card or Spell/Trap effect, but cannot be activated during the Damage Step.

activation condition means that this effect must be chained directly to the activated spell/trap card
