Stealth never works in this game....

My stealth tree is nearly maxed out yet I still struggle. Everytime you get ♥♥♥♥♥♥♥ close enough to somone they turn around at the last second. Every. Time. I dont get it.

Use the "predator" skill, or stay crouched all the time until you press stealth kill button. A good tactic is to alert a monster to make him go checking where you stand, while you go hiding behind something nearby. When the eye icon disappear hunt him down easely from behind.
Some monsters have stupidly insane perception, like the flamethrower dude.

Walk slower, but also there are a few enemies in the game (not a lot, like...3 or something) that are programmed to spot you the first time you sneak up on them. Might be that you're constantly trying that one haha.

I have no issue using stealth in Survival and Nightmare Difficulty. First, you will need Ambush Skill to hang behind the wall, and as soon as the enemy gets closer you, simply press the Melee button to stab an enemy. Once you save enough Green Gels, you will also need Predator Skill to rush and stab behind an enemy. To use Predator Skill, you must first aim behind the enemy without being noticed, then quickly press the movement key and immediately press melee button before an enemy sees you. But to use that Stealth Kill effectively, you have to have react quick. Even if enemies saw where you're going, all you have do is kite and LoS (Light of Sight), take cover behind the wall and stab them for good. Easy Stealth Kill.
