G-Sync seems to be working in my setup. In the nVidia control panel I have enabled G-Sync for both full-screen and windowed. In the nVidia control panel 3D settings I have Monitor technology set to G-Sync and Vertical sync to On. In The Evil Within 2 I have Vsync set to disabled.

Try forcing it to windowed and full screen in the nvidia control panel, it worked for me after the 2nd time, set v sync off in game and make sure in game settings are set to windowed bud!

Hope it helps.
I'm same problem here. Somehow, G-Sync is not talking directly between GPU & Monitor. I set G-Sync on and run in Fullscreen Mode but the game itself is behaving well with G-Sync. I even tried to set Displayed Refresh via Vsync Option but the game still runs at fixed refresh rate and my framerate was set to 120fps. So that is something that the developer and GPU manufacturer have to fix on optimization issue.

set nvida control panel to g sync / windowed and borderless. In game to windowed, then alt tab out of game and back in sometimes to make it work!!!
*Update*
Hmm, I actually figured out what's the problem. When I select Enable G-Sync for fullscreen mode, G-Sync doesn't talk directly between GPU & Monitor. To fix that, I manually select Enable G-Sync for windowed and fullscreen mode via NVIDIA Control Panel, click Apply, and worked fine. I even set V-Sync Off via NVIDIA Control Panel and it worked fine. So now my monitor dynamically adjusts real-time refresh rate, based on GPU rendering rate.

if you choose fullscreen option in game it actually runs in fake fullscreen(aka borderless window)

For all who don´t want to touch their global settings for Gsync in fullscreen only for this game, download Nvidia Profile Inspector at https://nvidiaprofileinspector.com/

Install, open, search für game profile and edit the option "Gsync Application Mode" to "Fullscreen and Windowed", save profile. So driver enables option only for this game. Seems like Bethesda game engines don´t like native fullscreen mode, why ever.
