No continue: multiplies your stylish rank by 1.2x
No damage: multiplies your stylish rank by 2x
Doing a mission using a Super character: multiplies your stylish rank by 0.2x, making it impossible to achieve S rank.
If you manage do get a no-damage run, you'll get no continue as well and it is guaranteed an S rank (it'll be your stylish rank multiplied by 2.2x). However, no-damage runs on DMD can be a bit tricky, since enemies take more time to die and have their own DT.
DMD threshold for S rank is 6000 stylish points, however if you manage to score around 5000 and have a no continue bonus, assuming you were hit, but didn't die, you'll be good to go.
Also, in DMD you'll probably be doing a lot of combos and in most missions achieving about 5000 won't be that hard.
HAH threshold for S rank is 5500 stylish points.
On HAH, enemies have the same attributes as SOS (son of sparda), so you'll be able to pull some nice combos too. However, there is (except for one specific mission) no no-continues bonus, as 1 hit means you're done.
Most missions are fairly generous with scores, so even if you don't get any bonus, you can still achieve S rank, provided you did some decent combos.
As mentioned before, some missions are not that fair and thus can be a real headache (namely m8, m10, m12 and m18).
I think that it's worth mentioning m8, specifically, since in all difficulties it can be quite weird for those looking for S ranks (at least it was for me in DH), because on lower difficulties enemies will have less health and you're not going to be able to do some neat combos (even though the threshold is less demanding [human = 4000, DH = 5000, SOS = 5500]).
and I don't think I have the patience anymore for a 90 min run through the Bloody Palace in 4, without a suspend save feature
