insane achievement not unlocking?

hey guys just beat your game (Good job with the game guys!) i was looking through my achievements and despite getting the "it's a conspiracy achievement" beating the game and according to my death count its at 20 i still haven't unlocked the insane achievement. is there something I've done wrong?

Edit: is it possible that perhaps the successful runs of a pyramid are not counting as deaths? i'm sure i've made more then 20 attempts!
