SPOILERS, obviously. I would hope that anyone reading this far in would be wary.

You can save Al and stop his drinking. It pisses off the Queen of Maggots, but you'll still be able to find a way to save yourself. You'll also be given an opportunity to save another character from death, the choice is relatively obvious. There are some character deaths that are unavoidable (to address the original post), but if you pay attention and make the right choices you can undo what has been done to them.
I can be more specific if you like, but that's the general idea. You can be "good" without having the game end prematurely and without ruining anything. You just have to figure out how to save yourself and everyone else afterward. I didn't use a guide or anything to get the good ending, I just paid attention to the dialogue and made logical choices and it all worked out. There are some hints in conversations that will lead you.
I did have one thing that I forgot to do before I "finished" the game but it hadn't saved so I quit and went back. I got an achievement for it, and it very clearly made a story difference, but I don't know the exact requirements for the best ending.
Darkstride is correct. You don’t have to listen to the queen of maggots and drive him back to drinking to save them. What you do towards the end can reverse that. I made a couple mistakes and got both achievements (one for making him not drink and also the one to make him drink) which was odd but I got the golden ending anyway. Guide not necessary. Go with your gut. A “good character” (not gonna spoil that) shows up and that gave me a lot more confidence that I was doing the right thing by not listening to her. That level is so good, though, because you’re supposed to feel conflicted, ya know? It’s a heavy chapter and I really liked it for that.
Correctly summarized.
...at least it sounds more forgiving than Downfall, where you needed to tick like... every box perfectly for the Golden Ending.
Did you also place the rose that you pick up with the first train wagon on the correct coffin? Once you place all items on the coffins correctly you'll trigger an event in the same scene as the graveyard which automatically gives you the key.
Is (Spoiler for the last chapter/last controllable section) ...do you actually get to wake up Jimmy and there's not just a corpse? ...because I'd have to play the whole game through again to know the answer and I'd kinda like to know if the alternate conclusion is more satisfying.
I still got the good ending even with that snafu, btw.
