That's up to you, if you want to min-max then you can leave them not unlocked but with custom mode you can pick what you want in the pool of random items anyway so it's not something that would "doom" your save.
Most of those posts are from before the custom mode was added, and for people that want to play with a limited selection of weapons and power because they want specific builds over and over it was a problem, but now they just need to play in custom mode and it allows you to change what specific build you go for as well.
I was always more of a fan of unlocking everything and coming up with a plan on the fly depending on the starting weapons and early drops, but that's because I play for fun rather than to win every single run.
Look around in your cell, there should be a door leading to the clothier that will allow you to pick your outfit.
I just don't remember if it appears before you unlocked an outfit.
2. Yes, managing your pool used to be a huge concern. These days this is largely an obsolete idea since they introduced Custom Mode. Once you kill an elite in the Ramparts and you absorb its rune you can customize your loadout from the main menu (Play > Custom Mode). So if you unlock an item and hate it, you can turn it off again. Personally I advocate heavy use of Custom Mode, but there are purists who don't like it. I think it improves the game a great deal and allows you to experiment without regretting certain items forever.
What remains is prioritizing how you spend cells. For the first 10 hours or so I wouldn't worry about it and just do what you think is fun. But long-term you will need thousands and thousands of cells, so if you spend them on unlocking bad items you might regret that later by having to farm more for the stronger upgrades (like filling out the Legendary Forge you saw after the Concierge). Thankfully you will later also get ways to farm cells quicker, so I wouldn't worry too much about it for now.
3. Outfits are purely cosmetic. In gameplay terms - they're a way to spend cells for people who have already unlocked everything else. Once you unlock the first outfit, the tailor will open in the starting area, allowing you to change outfits before every run without any limitations. Outfits are also special in that unlocking an outfit at the Collector will not immediately put it on.
2) False. That's completely up to you and there's benefits to both approaches.
Unlocking only the items you like:
- you'll probably get further in your runs
- you might ignore unlocking a very good item that sounded bad on paper
- you can miss out on discovering new loadouts and new combinations
- you might get bored playing with the same pool of items
Unlocking everything:
- opposite of the above
- you can get tired of coming across a lot of items that you simply don't like
- you'll spend your cells on unlocking those items; those cells could have been spent on your legendary forge (you can break down the Collector's door, if you didn't know)
I'd recommend unlocking the items that you think will be good and making sure the item pool is big enough to keep you motivated to keep playing. Then save the rest of the cells for your Legendary Forge and max it out. But it's up to you.
3) Outfits are completely cosmetic outside of one instance*. You can change outfits in the skinning room in the starting area. I recommend saving outfits as the last blueprints that you unlock because they require a lot of cells. Some of them are pretty cheap though.
*) On the highest difficulty of the game (5 Boss Cell) you need to wear a certain outfit to get the true ending of the game.
