The trouble with this achievement is that only direct kills count, collaterals don't. So if your wrecking ball has an arrow or bomb affix, and the arrow / bomb kills an enemy when the ball is thrown, then that kill doesn't count against the 5. If the ball doesn't quite kill it but then a following dot effect (like fire) does, it may seem like a valid kill but it doesn't count against the 5 either.
i did it in the Ossuary, looked for a place with two Spawners close together, let them squirt 3-3 Corpse Juices (or Corpses Juice? :D ), and went for it. Still it was only the third or fourth attempt that worked.
My guess, which someone mentioned in that other thread you replied to, is that getting hit during the combo interrupts the count. Which is weird for a green weapon that always has "Cannot be interrupted by an enemy's attack". If this is true, I still think it's a bug.
I'm not sure about the mechanics of the "cannot be interrupted" feature - I had situations where such an attack was interrupted, with several weapons. So I've kind of disregarded it for a long while now :)
