End Of Gollum?

Gollum, the guy in Toxic Sewers who asks you to get his rune for him, that is.
I've been playing the game for years, but I only just now read that you can find a room where he died. Is it just a really low chance, or does it only appear once, and if you didn't see it that's it?

Just a randomly generated lore room. Has a tombstone in it along with a treasure chest. Have seen it a handful of times.
