The overall plot is that the there is an island with a creeping plague. The King hires all his companions, courtiers, and guards to find a cure, to know avail. The higher ups are almost all what remains of your royal court. The King was, and still is, a big ♥♥♥♥. Throughout the game, there are hints that you, the beheaded, are the king himself.
The timelord gets involved to keep you locked in a cycle, but eventually bungles it and lets you possess your old body. Doing so, and reaching the hidden boss allows him to drink an elixor which lets him keep his body.
The last DLC is against your wife, who doesn't want you, or anything else, leaving the island. Unfortunately, it's revealed at the end of the game that it's too late; and the plague has already mutated things off the island.
