So what's the difference between 4 and 5 BSC difficulty?

So I'm trying to update my guide, I learned how to datamine some cool info recently and I'm trying to incorporate it into my guide and then I come to the difficulty level and ending up scratching my head.

I don't have Hell difficulty unlocked yet, but what's the difference between Nightmare and Hell? Because I'm now comparing mob tier for each zone of those difficulties and they are exactly the same. For example Prisoners' Quarters has 6 and 6, Cavern 34 and 34, Castle 40 and 40 ...
Both have AgressiveMob: True (teleporting enemies), extraElitesPerLevel: 2 and extraMobDensity: 0.25. So it isn't exactly harder.

Is there something I am not aware of? Is there a different enemy pool in Hell compared to Nightmare?

who knows, but the game is poorly balanced in its current state, even getting the 5th boss cell requires just slamming your face against your keyboard untill you manage to get close enough to a perfect set of weapons and abilities to challange the giant, a single run to the giant with 4 health flasks remaining takes multiple runs and hours and hours of careful play, and he can basically 1 or 2 shot you if you make a mistake, 70% of the weapons and skills are useless against bosses, the game has gone beyond challenging and turned itself in to an supremely frustrating slot machine on 4 cells.

The new stuff in 5BC is a new area at the end of the HoTK room and a new boss (which is familiar but new).

also the new area's name is the Astrolab (the big observatory in the background of the HoTK arena)

Yes, I know about the Astrolab but if that's the only difference then why do we have to access it from 5 BSC when it seems literally the same as 4 BSC?
