Death by curse when I wasn't cursed?

It's happened several times now. It usually happens when I get really far into a run and will randomly die in one hit. The last time it happened in the Promenade of the Condemned. The screen will say "Death by curse" but I hadn't opened a single cursed chest or broken a paid door.

Never heard of this bug, I assume you didn't have a cursed sword.
If you opened a cursed chest, chances are the icon just disappeared and you forgot you were cursed, but if you didn't, I have no idea.

Did you have a mutation where food curses you?

I notice sometimes the indicator above my head disappears when I’m still cursed. Anyway, picking up a cursed sword is my best guess here.

I've seen that happen if I leave a level and still have a curse. It re-appears as soon as I kill something.
