edit - you press the right thumb stick to have your head fall off, and interact with the fountain.
I was just going to ask this. I am stuck there with seemingly no exit after beating the hand of the king. Do I just quit the game and restart to go back?

What you're supposed to do after beating the Hand of the King in future runs is to detach your head and use the fountain to end the run, just like after you kill the King for the first time.

As for the door behind the throne, that's most likely for something new in a future update. Perhaps maybe a new end level and/or new final boss. Kinda like the Dark Room and Chest in Binding of Isaac: Rebirth
