(SPOILERS) What do I do after beating the last boss the second time onward?
After the first time I beat the game and was left as a blob it's quite clear I could exit by the sewage by hitting RB. But the second time I'm just left to run around with no way to exit. Do I just jump into the spikes or hit restart? Or is there a "correct" way to end my second winning run?

You unlock the ability to "exit" your body after you clear your first run.

You can do it anytime with R3 (pressing the right analogue stick) or V with a keyboard, and you need to do it to enter the exit at the foutain after you defeat HotK to end a run.

Does it do anything else? i've been trying for a long time to use it on other stuff to no avail

You can root and damage ennemies with it. I didn't find any other use, but I didn't try really hard.

There are a few secret areas that require the head, including a blueprint in the starting cell.

Hint: Look up, directly above where you start when you can control your character

The other areas that require the head are the one where you can't see anything and everything seems to be covered in spikes and at least one where you need to go through a 1 high hole that is at the top of a wall to reach the mechanism to open a door (impossible to roll into a hole that is directly below the ceiling).

Other than that (and the damage/root), you can use it to grab things while remaining in a safe position (if a blueprint or food fell into spikes for example).
