How to enable Nightmare difficulty?

Kill the Hand of the King. Pick up the first boss cell from him and inject it in the glass tube under all the items in jars in the starting area. Kill the Hand again for the second boss cell and inject it. Rinse and repeat until you have 4 boss cells injected - congratulations, you are now playing in Nightmare mode.

So in this patch you no longer can gain boss cells from other bosses, only from King's Hand?

yep, beat him on the hardest difficulty you currently have unlocked to unlock the next one.

Beating HOTK is way harder than assasin. I couldn't beat it in my new save in the normal difficulty so I shamelessly went back to public version, get 3 boss cells and get back to beta.
