What's the new world map?

I didn't play the game for several months, then I tried again yesterday and there's all these new cool updates and locations. But it looks like some locations have been moved around, and now I don't understand what's where. I reached the Hand, but I've not seen how to get to Ancient Sewers. And where's Conjuctivictus? Is it still in the game?
