The Ice Bow can pair quite well with melee weapons as well, I like using it with the Frantic Sword as it's a Tactics sword so I can stack Tactics on my weapons and actives to great effect.
Two shields are usually junk. You won't be able to put out parries any faster and you won't be putting out better damage.
For sure this. The other one being the throwable torch + any bow that does +dmg to burning targets. The "+dmg when no enemies are near" can make for killer all-ranged loadouts, and the "+dmg when parry/stick an arrow" makes for great melee + shield/bow loadouts.
What I'm really curious about are two-melee weapon loadouts. I guess using one that can apply bleed/fire/ice/poison could be useful.
Has anyone tried the stun-hammer + the assassin knife/rapier? Seems okay, but wouldn't help at all for any boss fight though. =/
Problem with something like that is, you can probably use a skill for a stun effect more efficiently.
The only melee weapons I've ran together was probably some boots, and anything else.
I've used a torch once, but it was lucky because I had a blade that caused more damage if they're on fire.
I do use 2 range weapons more often though. When I'm doing something and wanna play safe... I'm a big fan of the Hunters Bow for long range safe distance. Then I have like a crossbow repeater or Heavy Xbow for close range

The Quick bow crits if 2 or more arrows are in - I've used it in synergy with Hokatu (or w/e the bow is called) I mark the target and the get a crit after attacking after with the quickbow - Since it already has an arrow in the target
Two knives somehow count for the Quick Bow, so you get instant crits while the enemy bleeds from the knives.
They synergize really well together and cover each others weaknesses. The Wrenching Whip can be spammed whereas the Infantry Bow has finite ammo and the Whip draws the enemy in for that sweet, sweet crit damage.
The War Spear is fantastic for groups of enemies but has a slow start-up and is terrible against flyers. The Balanced Blade is perfect if you are only fighting one enemy or just need some quick damage. This also has the benefit of range, it's attack range being great enough that you are too far for other enemies to retaliate.
I've never thought of trying anything else so I don't know what works.
I got a Spartan Kick once that did 175% to frozen foes. Which is pretty powerful since it's a slow single hitting type attack. I had two freezing sub weapons to go with it and a freezing blast main weapon. That type of loadout made killing enemies without taking damage easy. Safely one shotting most enemies for a majority of the run. Though the loadout was below par against bosses because after 3/4 of their health is gone, getting bonus frozen damage was basically impossible.
