Report a translation problem
It happened to me on three different playthoughs. The RadRoachs just kill everyone no matter how heavily armed they are. This cheap act of god destroys all the time you put into it by cheating you. It a nasty feature that turned me off the game. I was hoping they patch it out. If I have the resources, people and the weapons, I should be able to withstand any situation. The game kills you off because it chooses to. It's a cheap cheat!

The easy answer is make a layout where there are some empty floors between the rooms that are almost always empty(Overseer, Storage, Living Quarters, spare power buffer room,etc). Incidents won't propagate to the occupied rooms. Also, these separated floors act as buffers to incidents, that is they have chance of absorbing a lot of the incidents.
Finally, incidents don't propagate diagonally, so early on you might want to stagger or zig zag rooms for awhile. If an incident occurs, no need to fight it, just move the dwellers out.
Radscorpions are actually very easy if you prepared for them before hitting that population mark (see link above).
That's why you: 1. keep your population under 35 till everyone is armed (when MoleRats are unlocked) & 2. don't have too many pregnant women in the vault at one time (and don't have more than one assigned to a room).
Once again, you do not have to fight incidents. Just move the dwellers out of that room until it's over.
There is no luck, just knowing enough about the game and planning ahead. Old screen shot, this isn't my favorite layout anymore, but it gets the point across. Not sure you are reading the information I posted so it's picture time....

It will keep you guarded and slow your expansion to a reasonable lvl. (increasing room level also increases attacks)
other than that just always have someone exploring to find weapons,I usually send someone out till they die then revive and return them, no damage on return trip.
