Insta-killed by grenades?

This seems to be something new since i played the game on mobile years ago.

Is there any way to defend against this?
I'm playing on survival for the increased difficulty, but this seems a tad excessive, just had two settlers get one-shotted by a sudden grenade while on a quest...

grenades do a lot of damage so make sure to heal everyone when you see the enemy throw a grenade. this might not be possible always because there is no healing-icon above their head unless they get very low on health. you have to click on them and manually select the stim pack in their character screen to force them to heal themselves.
