2. A valve in the cab for brake controls is closed
3. You have 2 locos (or more) connected as MU. Each of them you're not operating from should have that brake valve in the cab CLOSED and in the one you've operating form should be OPEN.
I've got the same issue with a DH4, brake line valves are closed, hand brake released. Brake air reservoir at 8 bar but the train brake line does not charge and the indicator keeps flashing.
