Why does the DH4 have a Car horn as the bell?
Is it just me or does the bell on the DH4 sound lerality sound like a car horn? Is this a clientside audio issue or not an issue at all?

It was just a design choice to make it different. Probably the label should be changed to not say bell though, even though the functionality is the same.

Now all locomotives are either European or converted to run in Europe and there you don't use a bell.

Converted to Europe... then why the logic of control levers remained (or gone into) american - style?

As long as all wagons are set up to drive in Europe with European couplings and all, there are no options to drive American. Even the signs at the track seem mostly British to me.

I believe the game is based on Czech railroads but I may be mistaken.

Fun fact: If the DH4 is loosely based on german V90 or V100 series (what the outside form suggests) they actually have a car horn installed. It serves as an alarm in case the engine gets cold enough that the coolant may freeze and crack the engine block. There's a button to test the horn (and some of the drivers like to fool people with it).
