1. Throw the fishing rod (if the fish is not visible, repeat point 1).
2. If the fish is visible, wait for the exclamation mark ("!").
3. After the exclamation mark ("!"), press the action button quickly and often (the "X" key by default).

For the mouse click the bobber area when you see the ("!") for best results.. But x is easier.
