Do you keep chaos emralds through playthroughs on save files

No. If you start a new save file, you must collect them again. It's just like how it was in Sonic 3.

That's not what I ment at all once you beat the game do you get to go back to the start with the chaos emeralds you have and then collect the rest

you can collect all of the emeralds after beating the game by resuming your save on any level and going to the special stage rings.

Each save file keeps it's own set of chaos emeralds. So once you get all 7 on that file you can use them as long as you don't delete the file and start over. Other files though will start from scratch and you'll need to recollect the chaos emeralds on them. The no save file also starts over everytime since you cannot save on it at all.

Blue Sphere medals are the only thing that you need to do only once.
