Secret (???) locations?

I can't find the 1st, 2nd, 5th, and 6th for The End. Could someone only tell me what screen I can find the entrances to those? For example, say something like "The third ??? location can be found in The End 8 ." I am asking this here because I want to generally find it on my own but just get a slight nudge in the right direction.

You don't as far as I know, and that is just a piranha enemy, you have to deal with them later on. The actual tumor is in the screen above.
