Right click on your desktop > Display Settings > Scale & Layout > make sure the scaling percentage is set to 100%.
There's also a scaling slider in game under the same menu as the resolution setting. you should make sure it matches.
may or may not be it but it did fix that problem for a friend.
Mess around with Fullscreen and windowed settings.For me, I changed it once to fullscreen and this fixed it for all settings.
Now that I have fixed it, I did also notice it was only after I had done windows updates for framework and stuff that it started acting weird. Not sure if that had anything to do with it, but it was fine before those updates for windows. Maybe keep that in mind for reference if you have any windows updates and BDO acts a fool to ya lol.
