Look at the games I play.
One of my most played game is Vermintide 2 and the only thing keeping me there is constant extreme challenges that lead to wipes more often than wins.
The early game in BDO felt the exact opposite, I push a button and everything around me explodes, it's so far beyond boring that someone would have to invent a new word for it.
But the reason I made the post is because I want to give the game another chance and so far I have been in a few boss fights I don't remember from last time I played and it was pretty fun, so fingers crossed.
NOTE: The main story line quests and a lot of the side quests are generally easier than going out on your own in more difficult areas.
Combat side quests don't start to get difficult until you move to the more difficult areas.
BDO is not the same game in any way, but I always like to compare it to the original Baldur's Gate 1 and 2. You are free to go any where on the map, but some map areas will take you out in an instant if you are not prepared for it yet.
After a point BDO does not hold your hand and give you the gear you need to move to harder areas. That is left to the player to figure out and do.
Already mentioned in this thread by others, but things of note.
Dark Rift Bosses: Level I bosses are easy like main quest bosses. Level V versions are more difficult, but can be easy if you finally got yourself the Higher-Tier gear to make them easier.
World Bosses: Nightmare, Stormbringer, Thundercloud, and so on etc.... are harder to fight than their normal versions.
In some map areas there are area specific champions. They are usually more challenging than the cannon fodder you are fighting. It is RNG if and when they show up and where they show up.
There is also a Mutant Ogre and Mutant Troll.
The only way you can take them down is with a very large party of players.
Those two are impossible solo.
Bring up your main map. The higher the AP requirement for an area, the more difficult it will be. Again, you do not have to wait to go there. You are free to roam the map. Just make sure you are geared for the area.
I know it is a Pain, but you always want to work on getting S rank knowledge for all monsters if you can. S rank makes them easier to handle and increases your RNG odds of getting better drops for the area. The better drops is the main thing you want.

What exactly is an NPC response? What's the criteria for that?
Most games aren't dark souls, let alone mmo ones. They always are easier than single players.
And without skill and knowledge of rotations in bdo, you will not succeed at grinding efficiently, and in later areas, grinding in general. The atoraxian dungeons will also prove significantly challenging to rush through without thinking.
