So if someone has "John Smith", you can't have "Smith NWesson" because Smith is already taken, even though they're using it as a family name and you want it as a first name.

Otherwise it's an art. Use compound words, homophones, letter substitution, a weird gamer name with the x's and such, etc.
You rarely see people with 'proper' names in game.
