So for example, if someone PKs you your guild will get a message that says "(Your Character) was forcefully slaughtered by (Their Character)"
And Character name is also used at the Mysterious Stranger to track players. So if more than one person has the same character name, they could have an entire guild hunting them down for something someone else did.
It is annoying, for sure. I just made a new character this week, and I'm going into very obscure names at the moment. But it is what it is.
Mainly the importantance of a second name is so two accounts having the same character name doesn't conflict.
The reason why I mean that a preset family name would work is that you can utilize this for further game lore and mechanics, like being able to set a 'family' relationship between players, similar to races or factions.
For instance a character can be of a Noble lineage in a human race, or a 'Villager' or a 'Sailor' or 'Pirate' lineage...
But for games with gender locked visuals and stuff, the customization for RP and lore is severely limited.
We won't ever see new AAA titles designed with RP and actually lore in mind, they mostly cater to the graphic ♥♥♥♥♥♥ and flash gameplay hyped players.

Elder Scrolls Online. :) Seriously, if you enjoyed Skyrim and Oblivion, give it a spin. There are a few differences for it being a MMO, but it's now more Elder Scrolls than ever – and there are people role playing in many taverns all day around. Plus it's dirt cheap when on sale. (You just have to jump that potential preoccupation many have from back at release and/or rumors.)
You obviously never played Tera the ♥♥♥♥♥♥♥♥ loli and prostitute infested MMO, atleast here the genderlocks actually make sense, bezerkers and warriors are male while archers and valkyries, tamers are female which kinda fits (unless your planning to make a trap/gay character then thats your own fault)
