When you launch the game without a controller plugged in it tells you clearly that the entire experience without a twin stick controller may suck big time. So yes, you need a controller.
Buy a ->microsoft<- 360/xOne controller (second better) and you are good.
There are better alternatives but are either more expensive or hard to find... with a 360/xone controller you can't go wrong.
It is relatively easy. The biggest problem might be some of the reaction mini games in the gym where you have to press a button that shows up on screen instantly (ABXY) and learning where they actually are (i struggled with that a bit back in the day on my Xbox360 because it was so different to the Playstation Layout i used before for so long over 2 consoles)
But yes, definitely get a gamepay. Get the wired Xbox360 one, it's cheaper than the XOne and gets the job done just the same.
I can't even imagine playing this on a Keyboard. I remember playing old sports games like FIFA 96 and 97 on a keyboard and that was annoying (Arrow Keys to controls, ASDQWE for the actions....) but today where you definitely need those analog sticks...no way.
If you got the patience to learn it, do it.
