Who is Paina?

I found this character on a wiki page but I don't remember seeing her in the endless mode in the original version or in Karamari Hospital.

She was a easter egg or secret In DOS-CAT along with the dolphin, lines, and the matress. It can be found in the orignal game

She is a reference to another game the developers worked on, I think.

I think she's meant to be the antagonist in a game that Akuma Kira may or may not be working on anymore called Akuma Kira's Day Off (or AKDO). He's since unlisted the tailer he made for it, and there haven't been any updates on its progress since 2014; and between Lost in Vivo and the fabled final dlc for Spooky's, I don't see how he could possibly have the time.
