Anyways, the issue is to make a map in English it is more work then it's worth. And the devs don't think it needs addressed so it probably won't be.
If your really committed though you can make a scenario.
Basically what you can do though is, Start from scratch (joy) or
1. Open the scenario editor and switch it's languge to English (4th option over, along the top. Close it and reopen it.
2. Pick the base game scenario you want to edit from (they are found in the game folder - Story.
- You can open the Story config file in Notepad or better Notepad++ and see what year each scenario starts, to know which folder is which.
- This gives you the officer list of that scenario (every scenario has it's own and there some differences but mostly it's who's actually alive (you can edit any of that of course).
- This also fills out all the other lists like units and such.
3. Anyways now go to your game folder - config - language - and open the English file. (This is the more work then it's worth part).
-First thing you need to do is figure out who is who so you can edit the officers you want to edit. To do this you need to use the numbers in the Surename and First name column and find them in the languge file, and manually translate. Such as officer 1 is 10100 and 10426 is A Gui.
-Once you know who's who etc, you can just change the other columns to what ever you want them to be. And your good to go.
The rest of the editor is pretty straight forward I think. If you can figure out who's who.
One way to do it is use the city list. Look in game what city they start in, then you can find them slightly easier in the list because you can see what city they appear in.
You can use their protraits. If you go to the game folder / res / generalicon you can see, for example number 37 is Cao Cao's portrait, and so in the editor you can find the officer that uses 37 and that's Cao Cao (and I think they are in the same order - so number 37 is Cao Cao).
Just remember you don't need things in the name column for it to work. The game only uses the name if it's a new officer(and you don't use numbers from the languge file to make the name). So you only need to change the ones you want to refind easier.

With the premise, Liu Bei fled to Ma Tang instead of Liu Biao (and so Liu Biao gets divided by Sun Quan and Cao Cao).. And add early Meng Hou just because... Why Not? ..
So its possible. But It's a lot of extra effort. And worst of all, is when they update the game it doesn't update with it. So if they add new skills or change units or something it doesn't change for your scenario since every scenario has it's own officer list and such to reference.

Manged to get a little bit done on it, moved some people around with out causing it to bug out to much. Year is 190, thinking about making a yellow turban force to set in Luo Yang but it might be easier to just make them as new officers then add them to the game i aint decided yet.
Changes
Moved Dong Zhuo out of Luo Yang
moved a few key officers with him but most i turned onto free officers and spread them across the map. I hope this will weakening him some and give some the other forces like Cao Cao a chance to grow.
Combined Ma Teng and Han Sui
Moved Sun Jain's forces to Wu
Moved Liu Bei to Jiang Zhou
These moves also made some space to put in some created forces to play on the map.
You can load the game with cheats on, then open the cheat menu find the officer and their avatar number is probably their character number in the editor
I checked it with some rulers and I think its correct. While still fiddly it should be a more manageable to find the officers.
After the yellow Turban rebellion failed Zhang Jiao and his brothers captured Xiang Yang and made it their new strong hold. Dong Zhuo sized Luo Yang but was soon forced out by the coalition forces and is fortifying his defenses in Chang An. Ma Teng and Han sui have come together to defend against Dong Zhuos assault. Liu Bei has set up in Jiang Zhou while Sun Jian begins his expansions in Wu. Many officers unsure of what the future may hold roam the land looking for some sign of hope. Who will survive this storm of chaos?

Though finding the numbers with the cheat menu might make it a lot more manageable to make one. I had 5 scenario ideas but the editor was to much hassle to make them before, So Maybe I'll mess with them sometime.
First i made Zhang Jiao a wife and gave them a kid but he becomes an officer the 2nd turn of the game no hammer what i do. I made him underage and change his status to under age but no matter the age i set him as he still becomes an officer on the 2nd turn of the game. i have set him to unborn and made his birth the next spring but still on the 2nd turn he becomes an officer at the age of 0. I have no idea what i am doing wrong or if it is just bugged.
<Edit> I figured this one out, its because i had him set to appear in in the city the force was in. I dono why it made this under age person become an officer because of that when it does bot do it to all the unborn and underage officers set in cities all across the map but when i put his city as blank he stopped becoming an officer on the 2nd turn.
I wanted to add some birth magic thats is not as much trouble to get but when i tried to use the Item maker to do it it does not have consumables listed . So I used the tab that lets you give items to a faction and I made a few one stack birth magics but didn't give them to any faction, I thought maybe it would let you find them randomly if they are not given to any one. However the game crashed on the faction pick screen every time i tried to star a new game. When I removed them all saved it and then started the game back up it plays fine.
Also has any one tried city editing? It says i can change the population and what not in a city how ever when i start a game and use my custom faction in that city the population and developments seem to be reset back to the starting numbers. Is this because you can not edit a free city? when i edit a city a faction controls and then start up the game it seems to be changed for them. Have you tried this? Know any work around?
Oh and you can load your custom maps stright to the work shop from the custom map page in game.

Also don't know the issue with the child but underage children will follow the parent regardless so should be fine to just leave it unassigned and turn two they will move to the parent city. I never messed wit adding new officers.

A different fix to birth magic, if you just want more children in your game, is to make it appear in the in town shop. You can adjust the price to be what you think is fair. To do this you need to..
Open the game folder/config/world/items. Back up any files you change so if you break it, you can revert easier.
Search it for "ID":581, and this is the item info:
You can change price to change what it costs in game. (I usually change price and cost, but I forget offhand what cost does).
Then to add it to the in town shop, open itemshop (same folder as item file) and add to bottom before the ]} :
I put it before the "ID":702 one so it's in order, idk if that matters. Just make sure it looks the same as everything else. one missed comma will make the game crash right away.
You can adjust rate to make it appear less or more often. Higher number is more common and idk what the max is but 750 is pretty common but you don't always get it every quarter iirc.
So i figured out that if you change a free cities stats and then start with a custom force in it the city's stats go back to the games base, how ever if you leave it as a free city and then start the game the city will have changed stats like you want it to.
I never messed with gates. If you wanna mix up battles, you can make tactic ranges double or more and the AI will throw around a lot more disorders and such before they are in range to attack.
Since most of the time free cities have lower stats, that makes sense I guess. Since they probably do that so you don't start with a crappy city. They don't account for you making better free cities.
Some KOEI ROTK's have options to randomize where officers appear but idk if they will add that here or not.
