That is a pretty broad question, but I'll take a shot at it.

Assigning skills:
Assuming that you created a custom character (and not a prebuilt kit), your character will have a certain number of skill points based on class and intelligence score. To assign them, click on the little juggling icon at the lower right corner of your screen (or press X, by default). Click on the character you want to adjust in the list on the left side of the menu that pops up. Scroll down to the skill you want and press the + icon to add ranks. If the skill is a class skill for your character's class, then 1 point equals 1 rank, and your maximum skill rank is equal to 3 plus your level. If it is a cross class skill, then your max is halved and each rank requires 2 points. Rogues generally get the most skills and have the most class skills.
Note: If you made or hired a character who has a prebuilt kit, your skills and feats will be assigned automatically at each level, and they usually don't put points into skills that require training.

Ranks vs. Proficiency:
Some skills, such as lockpicking, disarm traps, and most of the mining and crafting skills, have a proficiency system implemented in low magic age. For these, the system is a little more complicated. Your number of ranks does not directly increase your skill modifier. Instead, it defines your maximum bonus. To get those skills to rise, you have to assign skill points as usual, and then you have to make the character use the skill (go pick some locks, disarm some traps, mine some ores, or build some weapons and such). Each time that character has to make a skill roll (whether they succeed or fail, but success is better), they gain some proficiency experience that helps fill in their proficiency level for the skill, up to a maximum level of the number of ranks invested into the skill. Your total skill roll for that character will be D20 + Proficiency Level + Relevant Ability Score Bonus + Miscellaneous Bonuses or Penalties.

Untrained Skills:
You may have noticed that some skills appear to be locked (grayed out), and you cannot add skill points to them. Before the character can rank up those skills, it must find a trainer to teach it the skill. If you are playing in Adventure Mode, playing through the campaign quests will eventually lead you to cities that have trainers, which you can pay to do teach your characters.
