Things such as staring at unsettling scenery, the monsters, or standing in darkness for too long will degrade your meter further. If the meter dips down too low, Daniel falls to the floor and undergoes a nervous breakdown for several seconds before recovering. Apparently your flailing around can also attract the unwanted attention of Gatherers, but that's never happened to me in vanilla since most of the monster encounters throughout my game have been heavily scripted and situational. I only collapsed twice, and that was solely because of the imagery Daniel saw in those specific rooms near the game's climax.
It's a pretty neat mechanic that has potential, but it's rather superficial and just ends up getting in the way.
