Maybe it's just me being unsatisfied with the ending but what Tsumugi showed you in the beginning in the game really bothered me, if she cannot dress herself up as non ficional characters without breaking out than how exactly is she able to wear her own clothes? Her statement also completely contridicts with the conclusion that every previous Danganronpa character is actually fictional because if that's true than why did she break out when putting on Kaede's clothes? Seem's to me that if we assume her cospox is actually real and not fake than she lied about what exactly it reacts to, it's actually the other way around. She cannot dress as fictional characters and can only dress as non fictional ones. This coincides with the subtle writing near the end of the game where Tsumugi seems stubborn and ignorant about that idea that fiction can change the world despite what her entire character has seemingly revolved around, obsession and appreciation of fiction as the ultamite cosplayer. Despite all this I still find the ending frustrating but I think you can come to the conclusion that the Danganronpa characters at least the ones previously before V3 are actually real .

It's made to be ambiguous whether anything Tsumugi says is a lie, fitting in with the theme of the game, truth vs lies. It's like the ending to the original game. Same as the first game where you didn't have to believe anything Junko said, it purposely doesn't show you what's outside of that dome so that you can choose to believe whatever you want. You can choose hope or despair or anything in between.

It has been a bit since I went through the ending, but aren't they super explicit that the first like three danganronpas are fictional, a game or show or whatever, and the rest up to the current are "real"? As for the Kaede thing, it is blurring a line for sure, but despite the Kaede character being made up, she is also still a physical person that exists in the real world, so you could argue she is cosplaying the real person there. The Tsumugi character doesn't have a real person stand-in, so she is able to cosplay her.

There is a really strong implication in the game that Tsumugi can't actually cosplay as fictional characters, and instead can only cosplay as real people. During her introduction she says she prefers making cosplay clothes instead of wearing them. During the first trial (I think) she says there are "people who prefer cosplaying as real people", which she says is "unforgivable". During the ending of Dangan Salmon Team, she explicitly refers to Shuichi's clothes as a "cosplay" (the exact context is that in her Free Time Events Shuichi tells her he's interested in trying out cosplay, so in her ending she says that "it will be an even better cosplay than this"). While you may say this isn't canon because bonus mode, don't forget that School Mode and Island Mode also made heavy references to spoilers from their respective games, such as (School Mode) Junko implying she's not actually Junko and Chihiro telling Makoto he's a boy and (Island Mode) Byakuya telling Hajime he's not actually Byakuya and Chiaki implying she won't be able to leave the island with everyone else.

There are many hints all over the game that point towards all the games being canon. It's more noticeable when replaying certain segments (like the trials).

From the way that the characters from the second game act and what's confirmed there, they were a part of the ultimate despair so they were also fake right? The only real people are the characters from the first game.

No, the memories implanted into the V3 cast contain a lot of inconsistencies, including inconsistencies related to the Remnants of Despair. If the cast from the first game was real but the cast from the second game was fictional, Tsumugi wouldn't be able to cosplay as both regardless of whether her cospox affects cosplay of real or fictional people.

Honestly, im playing through the ending for the second time because the trial is so confusing and i still am iffy about it. the ending feels so weird and rushed i dont like it one bit
