it's more the sexual nature of some of the commentary and the suggestive themes, aside from that I can't imagine it's really all that bad, I mean, hell, alot of final fantasy titles still have to put up alcohol references in their rating information when some of them, it's just the use of potions, lol

Its pretty harmless stuff really. Characters say "♥♥♥♥" and "damn" and there is quite a lot of adult humour.
Game is "age 16 and up" in germany btw.
Different sensibilities i guess.

There's a (spoiler for early game scene) sexual assault scene a few hours in, that probably contributed a lot to it.

I don't know how many FF games you've played, but ever alcohol warning in them has been for actual alocohol.
Don't know where you got the idea they could get away with that warning for something it doesn't have.

There's a lot of "grotesque" scenarios that could be rated M depending on who you ask. Such as Killing tiny gnomes when pressing switches and leave bloodstains because you literally AoT their level., read people getting stoned to death complete with voice acting and an NPC getting eaten alive after you fail to save them written in a way that can probably invoke feels. Just to name a few. There was also a sexual assault scene with a specific character that resulted to the main witch character even throwing up.

Your character swear if they miss. Even if it sounds banal it took me by suprise to hear it in a JRPG
