This was enabled from the beginning how do I turn this off?

Open console and type cht_bEnableCheats = 0
Then load any save. It should work.
One note: If you save your game with cheats on, that save will enable them on loading.

Is there anyway to start the game with these turned off or do I need to do this each time I start a new save file?

You can open console in the menu and type commands before starting/loading a level.

Yeah, sometimes the console command won't save. Just try typing it in main menu and exit the game. Check later if the command is disabled.
