lol but no really you haven't mentioned last resort so you probably didn't try that?
i did that
although honestly, with dgvoodoo2, last resort isn't even needed for high resolution
i set the game to 800x600 and set the dgvoodoo2 resolution to 1920x1440 and i'm good to go (i could play it in 2560x1440 i just have no desire to )
reoccuring issues though:::::::::
the mouse flickers on and off but i found that at least setting the game resolution to 800x600 made it much less noticable
the sound regularily has a crackling sound for reasons i can only imagine
there are stupid bugs which i have to assume arent' in the game when played on its inteded system (win 98) such as getting stuck at the bottom of stairs and a whole host of AI bugs even on the game's standard vanilla maps
ultimately, SWAT 3 is masterpiece and super addictive but there is a lot of bullshtt tthe player has to put up with, at least on windows 10...... i doubt its actually any better one windows 7 though
id like to try the instructions on the last resourt website for playing the game with wined3d but even creating multiple installations of this is a major ♥♥♥♥♥♥♥ pain in the ass...... move the files from one folder location to another and you break the game
i wish to god the source code had been released, then this masterpiece could have sort of lived on forever as a "must play" title, a right of passage..,.
but in its current state there is no way in hell i can recommend it to anybody who isn't nostalgic for it
Windows 8/10 are the source of problems for framerate issues on old games like Swat 3. Windows no longer supports 16-bit colour mode and emulates it in 32-bit colour mode. Blaming Swat 3 or Steam or GOG.com is unfair.
I don't have a problem with dgVoodoo and Swat 3 in terms of flickering mouse or anything you describe. Do you have an Nvidia or AMD card? I have found that AMD cards aren't as friendly.

Some people also find older versions work better ie dgvoodoo 2.54
