What's up with the difficulty settings in this game?

So I started the game on medium difficulty like I usually do when I try a game I've never played before. Guys are one-shotting me left and right, they are crack shots with almost no reaction time. So after two levels I switch to easy to avoid the frustration of having to savescum (which was doubly annoying since there's no hotkey for saving or loading.) Now all of a sudden they can't hit the broad side of a barn, no enemy can reduce more than one heart of health with an attack, most not even that.

Whats with the extreme difference? On easy, having played five levels the only thing that has managed to kill me was that wind tunnel in the canyon, never an enemy or even the bosses.

I'm playing on bad and yeah it's pretty rough. Constantly getting 1 shot. Kill 3 guys, save. Repeat. Still fun though, you just can't run and gun.

I got much better at this game, they have a lot of trouble hitting moving targets. Play it like doom
