I believe Tony's son died the same way - he played around on a bridge that collapsed into a river. His daughter however for some time experienced severe guilt being present and unable to stop or save Benny (IIRC), and thought Tony was punishing her by avoiding her even though it was because he was mourning and seeing her reminded him of Benny.
There was a clue in the boy's notes south of his bed - the medication is primarily used for lung cancer. It was also revealed by the end that the boy was bald which is a common side-affect for treatments like chemotherapy, for cancer. It's unclear how the boy got cancer (not unusual for real life), but I don't think it was from the Fukushima disaster as it seemed he got sick before the tsunami hit....and falling ill so fast from sub-acute exposure doesn't happen with nuclear/atomic radiation.
Uma was seen on the ship indicating he had also died although it's unclear to how and when. Since he was elderly and apparently left his family and lived on the streets/in poverty he most likely would die from exposure or poor health, or possibly the result of the earthquake/tsunami.
