Report a translation problem
There is a book called "Unaussprechlichen Kulten" (won't spoil too much here), just read the book closely and you will get enough info to be able to start the event.
It takes place in Otradnoye outskirts....
Unfortunately I can't remember where I found the book -.- , probably in a random encounter...
When you kill the book haters for the bookseller, there is an option to get An interesting book instead of usual payment if I remember right thats how I got the book

I like the idea,
doesn't have to be neccesarily a bonus to the character imo, but a trophy (like a tentacle^^) to decorate our personal bunker would be a nice addition. Just something beyond the xp to remember.

"you hear voices" would cause you to pause for ten seconds and have a conversation with yourself. in combat this would take a full turn, out of combat you just mutter odd phrases and people look at you strangely.
"panic attacks" would cause you to run in a random direction for a turn in combat. same thing out of combat, for 5 seconds of real world time.
again, very rare effects; might happen once per 24hrs on average.
...and you can't get rid of it. if you try to sell it, drop it, trade it... the voices won't let you.
on the plus side, the trophy would add say, 5hp, and you would be "fearless" against any normal thing that might cause fear (what's scarier than lovecraftian horrors?). you don't wear it; all the effects happen simply by having it in your inventory. you would get the "Servant of Azathoth" appearing as a distinction on your character sheet.
there should be a warning before removing it from the inventory of the shog:
"You see endless visions of otherworldy horrors when you touch the shog, but surely a trophy from something like this would bring a great price?"
What to do is more or less explained in the book. I you gave up solve it:
You need got at stone spiral, myself I request companions wait out of the spiral, I have the feeling they can block the spawning. I also wait midnight even if it's probably not required. Then I eat 3 toadstool (2 is perhaps enough). Open the book and browse the pages, then close it and close inventory, then the monster spawns.
I never succeed at first attempt, either there's a detail I don't get and apply randomly, either the spawning is quite sensible. At least you can try as many time you want until the spawns happens.
