I'd bet he is real, but like in other dimension or something of the sort since he can kill you. The question here is; Does he kill our material body in "his realm/dimension" or psychologically. Unless it is implied that the stone circle + toxic halucinogene mushrooms will force your mind to see something otherworldly as that thing and when your HP reaches 0 you die by having a mental brakedown/hearthattack... Still, even that is hard to confirm since the stone tool is the creature's bane + that doesn't explain the book too (implying we imagine that stone tool is an Excalibur). The book KNOWS exactly what will spawn there which means it's real, right? Or is that spot simply PERFECT for all Humans to see that thing when they get drugged by shrooms so some cultist made a book for it? But what kind of astronomical coincidance would that be. Pretty sure it's real. :D
