This is much better than Friends Only, because once one of your friends join, any of *his* friends can join as well, even though they aren't on your friends list.
Btw I'm a bit disappointed game do no let you exclude the home network from your joinable server. The problem is not friends, but ppl in the same network: they can see your game and join simply looking at the main screen in the "steam server group". It's weird.
If you don't want to use that mod, your other option is to create a lobby with whoever you want and then have everyone go offline in steam, using steam chat in browser (or you could edit text files in addon campaigns or something).
I understand and share your hesitation. However, in this case, I can assure you that it is unwarranted. l4dmaps is the "old" workshop - where all L4D/2 mods were hosted before Steam even had workshops. Rayman is a very well known individual in the L4D2 modding community - his mutation Special Delivery is one of the only community-written mutations that have been incorporated into the retail game, among numerous other accomplishments.
Also, vpk files such as the one from the link can be viewed with tools like GCFScape, allowing you to see exactly what is inside the vpk. If there was anything malicious, it would be very easy to see. And in this case, I have actually downloaded that file from that server and examined it to see what it was doing (I was curious how it worked, I mod this game myself).
You are correct that people can still find ways to join if they're persistent, but if it's set to LAN, certainly you can just ban the people you don't want from your server. I haven't tried it, but you could also try setting up a password, and just not telling the other person(s).
