That usually fixes it like 90% of the time.
If this happens in single-player or on a server you run, enable the developer console and type "sv_consistency 0" without quotes into it.
What now?
Well, not exactly. Official servers and most community servers will have consistency disabled by default, so if you run into an error it's commonly not because consistency is on.
Map errors (usually on addons) mean that you and the server have different versions; either yours is outdated or theirs. Most other consistency errors are due to one of two things; you have a poorly made mod which is interfering with your official game files and needs to be deleted, or one of your official game files is corrupt and you need to validate your files. Try deleting the weapons_smg.txt file in your /scripts/ folder and verifying; if this doesn't work, turn off your mods one by one until it does work, and then report the issue on the mod page and unsubscribe.
