How do I modify the controls permanently?
A while ago, I got this game, looked up how to make a controller work for it (because I'm one of those younger (23) people who grew up on consoles before finding PC), and I've been more or less playing happily ever since.

One thing I've had to do each time, however, is go into the options once I'm in game, and set the controls to trigger the voice chat, vote yes, vote no, message all, and message team keys. If I wait to do it before I've gone in game, then it won't register: once I join or start a game, the keys will go back to normal. When I do it in game, it stays the way I set it... until I turn the game off, at which point the controls go right back to normal.

Is there anyway to modify those key bindings so that the controls will always be the way I want them to be without having to fix them each time?

That's odd, normally those settings are saved. Try turning off Steam cloud sync for l4D2

I'm a "younger person" at 22, and I can barely even fit my hands around a controller, they give me cramps; all I've ever known is PC.

You can use this program, ive been using it for years and works and fit with your needs perfectly.

That program its used for set each button of a controller (even non-xbox controllers) to do a custom thing, for example with the joysticks you can move the mouse, or with 1 button press the right click, or a letter key or whatever you want.

The page its in spanish, dont worry, you got the download mirrors at the bottom of the page and the password is there too.

Put your controls and other settings that should not change into a file and call it autoexec.cfg and place it in the same folder as the config.cfg..... aaaaand you're done !
