1) its basically just the charger. it moves quickly at the survivors to deal high damage, it has certain damage resistances, and the terrain can be exploited against it. If it were added in l4d3 then both it and the charger would fulfil the same role, diminishing both of them.
2) conceptually, the idea of an infected growing armour plates and rolling into a ball to attack is a bit more far fetched than the rest of l4d2s relatively grounded mutations. however it could maybe work if, like you said, l4d3 was more of a post apocalypse setting rather than in l4d2 where the infection is fairly new.
3) making it resist damage on certain parts of its body could end up being annoying, since L4D2s weapons arent designed to be super precise (most of them cant even ADS) and the intent of them is usually just to spray and pray into hordes. Having sole exceptions to gameplay rules can work at times, but I dont think it would in this case. A good example is the Witch. She can't be dealt with by just opening fire on her, since she has too much health and can kill survivors in seconds. You either have to find a way around her, crown her, or use items like propane tanks to kill her. This works because she serves moreso as an obstacle in the map rather than a special infected that actively tries to kill the player. In the case of the sentinel, if it was similar to the other specials that hunt down and attack the survivors, then it would be the only one to have this specific condition that would require extra strategy, cooperation of items to overcome.
I think if you wanted to make it work, it would need to be a miniboss like the tank or witch, that wasnt guaranteed to spawn on every map, as well as potentially reworking the concept to fit more in line with the rest of L4Ds mutations
Distinct Roles: While both the Charger and Keratin Sentinel deal high damage, their gameplay mechanics can be distinct. The Charger relies on brute force in open spaces, while the Sentinel could excel in strategic positioning and terrain manipulation, offering unique challenges for players.
Conceptual Consistency: The Green Flu virus's high mutation rate, coupled with the phenomenon of hyper-keratosis or a more rare form; disseminated porokeratosis, where fingernail-like growths appear on the skin, could explain the origin of the Keratin Sentinel. This condition, accelerated by the virus, could result in the development of armored plates on infected individuals, fitting within the game world's logic and lore.
Gameplay Integration: Special infected with specific resistances can enhance gameplay diversity. The Sentinel's resistances would encourage teamwork and creative tactics, rather than being an annoyance, while maintaining the game's strategic depth.
Miniboss Rarity: Designing the Keratin Sentinel as a miniboss with randomized appearances would make its encounters impactful and memorable without overshadowing other special infected. This rarity would add variety and excitement to gameplay.
