Updates can break mods, but no update can remove a mod from the Workshop. When a mod mysteriously disappears, that probably means it was manually removed by someone for unknowable reasons. This is why i now back up my favorite mods, as well as anything that will get flagged.
If some mods are just disappearing, and others won't go away, that leads me to suspect your addonlist.txt is the problem. This is in /left 4 dead/left4dead, and it controls which mods are visible in-game and whether or not they're activated. Maybe it's read-only? Or i suppose if you rename or delete the file, the game will make a new one.
