Because it's a videogame.

But to note, spread is able to be affected by a lack of stability. Shaky barrel = wider spread pattern possibility.

In most videogames, crouching increases your accuracy based upon the notion that you're able to better aim by assuming a better firing position. When you're running around standing up, the crosshair gets bigger based upon the assumption its hard to accurately aim whilst running around or not holding the gun to your shoulder-stock position (even though thats perfectly doable).
The gap between "videogame realism" and real life is amusingly absurd sometimes. Hell, I was able to reload, aim and accurately fire a Paintball gun whilst standing upright running around. Not at full speed, but certainly at a jogging pace.
Some games are like 'Oh you're standing up and moving? You couldnt hit the broad side of a Sports Stadium'.
Games like Killing Floor do this concept properly, I reckon. In Killing Floor, all guns are ~extremely~ accurate, whether you are 'aiming" or not. Because seriously, a gun's bullets go where you're pointing the damn thing, it doesnt matter whether you're running around or not. The premise in KF though is that it is "easier" to aim whilst standing still or using the ironsights, because you dont have a crosshair. However, the gun is still as accurate as it always is.
The main problem with the crosshair-concept in most videogames is that it sometimes leads to totally (and I mean totally) ridiculous outcomes. In Counter-Strike for example, you can be standing dead-still and aim point-blank at a target infront of you, yet with an unscoped Sniper Rifle the bullet will magically turn a near 90 degree arc immediately after leaving your gun, due to the "size" of the crosshair and the forced-gameplay premise that its a Sniper Rifle, meant to be used only when stationary and crouched.
Shotguns are given ~huge~ crosshairs to represent their general inaccuracy and wide pellet-spread. However, it really bothers me how utterly inaccurate this is. Most real-life Shotguns are accurate for quite a fair range, certainly 3-5x how they are represented in games. I know, for a videogame, that certain concepts change for the sake of gameplay, but most specifically Shotguns and Sniper Rifles get the worst of it.
Also, strictly-speaking for L4D, it amuses me that Shotguns were ~so~ overpowered in L4D1, so when we got them in L4D2, they were nerfed into oblivion. Almost every other weapon in the game is superior to the Shotguns, whether the pump-ones or auto-ones.
In L4D2, Shotguns are useless against targets even medium-range away from you. They take a (compared to other guns) very long time to reload. Their penetration is limited unless you're at absolute point-blank range. Shotguns, compared to other weapons in L4D (kills-in-comparison-to-ammo-spent), also have very limited ammo supply.
Melee weapons cleave through 5-10 Infected per swing with a guaranteed kill and do ~immense~ damage to all Specials. Magnums kill any Common Infected in one shot. All Sniper Rifles kill Commons in one shot, unless playing on Realism. Most rapid-fire weapons reload quickly, do moderate damage and have moderate penetration, being effective at all ranges.
In my opinion, the best weapon in the game is the Hunting Rifle, especially with a Laser Scope. Keep in mind the Hunting Rifle is more accurate than the Military Sniper and you rarely need those 15 extra rounds per clip. Its accurate whilst moving. Kills all Commons in one shot unless on Realism difficulty. Effective at range or up close due to its extreme penetration. Large ammo supply. 15 shot full-clip reload. Scope if you need it. Decent bodyshot damage, kills most Specials in 1-3 headshots.
