Don't you love it when you have to pay attention every second in console & the status command to make sure it's only you & your friends. PHAK NO !
Seriously everyone , I would contact Steam support if this issue becomes a epidemic. Friends - Only lobby & games should remain just that. FRIENDS - PHAKING - ONLY !

No, you set the LOBBY to friends only. That does not mean others cannot join later.
What really makes me angry about valve and this, is that you actually don't avoid matchmaking at all, since it still controls who enter on your server (conclusion based on the fact that we still have reservation problems, even connecting directly with ip, i mean, last slot glitch or session no longer available errors still there)
I think its and error/glitch on the matchmaking system not checking for friend status before allowing the user to connect to the server.
I have been using the dedicated server tool provided in library-tools and never had problems like this when playing with friends.
