Ammo would be little bit more scarce and difference between tier 1 and 2 weapons would be less. Add different weapons for just varietys sake. You could take tier 1 guns with tier 2 guns if you want. Scout could be very strong gun against SI but due to fire rate its bad for hordes attacking you.
Make zombies little bit slower but tougher. Aiming matters more and game is in general harder. This would force you to loot for ammo more, and there could be ability to add laser for every gun, even magnum. There could be other modifications than just laser for weapons to be found.
Also, more of everything. More campaigns, weapons, and even bigger emphasis on team playing.
2) Proper hitboxes and hit regulation. Things like making the Hunter's/Charger's/Jockey's hitboxes not being ahead of their models, and melee swings actually hitting their targets rather than making a hit noise and doing nothing else.
3) Gmod-esque modularity, allowing for custom campaigns and mechanics to be added in easily.
_3b) Absolutely no skins, microtransactions or cosmetics. The client-side modding system is good as is.
4) A suppressor attachment and noise system. Commons are less aware in general but are more sensitive to light and loud noises. Gunfire and running, along with flashlights, draw more Commons and increase the spawn chance for a Horde. Ideally the Witch should also be made much more sensitive to being lit up and walked around.
5) A set of .45 dual pistols in between the Magnum and the normal dual pistols. 7/14 round capacity and mid-level power.
6) Tankier Tanks with no Special/Common intervention. The Tank should be a proper boss fight of its own, and not have its difficulty artificially inflated by having other zombies interfere.
_6b) The AI Director doesn't summon hordes while dealing with a Witch in close proximity. See in conjunction with #4, a more sensitive Witch would require time to skirt when not rushing to crown.
7) Casual and Competitive Versus modes. One allows mods and so forth, the other stops mods and uses a balanced config. Maybe a ranked matchmaking system. Let's avoid the stupidity of the last month.
8) Maps that randomize their layouts so the campaign gains a bit more life. Could even have different crescendo events on different paths.
_8b) Infected and Specials that have random visual differences between each other. Not every Hunter looks the same, and so forth.

Insurgency: Sandstorm has such a system implemented, and you can level untill level 1k its pointless but i like it, it shows everyones level on the leaderboard, and there you can see which people actually already played alot and so on.
